Did installing nova launcher stop the icons from disappearing? I have one fresh out of the box doing the very same thing. This is the second one I have had to do this, both made in June of this year, wonder if thats the common thread among all of them?

Worked for me. Using Google now launcher now

I found thru research that the updates to Google search can also cause any launcher to not be able to be set to default and stick. I had this happen to me with nova but once I uninstalled the updates nova remained my default launcher and the icons stayed in place thru every shutdown and restart so far!
