60C sounds pretty good, so I wouldn't bother with a hole.

Different if no lid !
I would use some double sided foam tape (normally cheap 3m type Chinese copy is good.)
I put small pads on each corner to mount on HS and thicker, small tripple layer pads on top at each corner - do not remove top backing layer so they don't stick to the lid.
Then gently screw on the lid to leave an air gap above the fan and round the edge of the lid.
If you can't screw on the lid try with small pieces of tape at each corner to leave the air gap.
See what temps are like in this set-up ...

Thanks to the kindness of dbramm22 I have just tried out his find of the ADDA AP3505LB-J70 - A Heatsink and 5V Fan all-in-one.

Lovely neat unit.
I have used a 50 Ohm resistor to slow it down to silent running ...
I have used 4 pads (Stack 4 high x1mm double sided foam tape) to space the lid away from the cooler
(also holds the unit to the SOC as I am using MX4 paste),
and covered the resistor and solder joints
I used the same idea for pads with the SZYTF Turbofan from AlieXpress

I have no test data for this lovely combo unit as my M8S ProL has decided to fail on me
Just flashes the Android LOGO a few times and then blank screens !
Probably just a cracked solder joint somewhere but ...
I am pretty sure this ADDA unit will be as good (or slightly better) than the Alloy Turbo Fan unit.
Looks like a quality item and has super low noise with a 50 Ohm resistor.
My best guess is that if you are in the US area then the ADDA is best, but if you can't get a good price on it where you are, then the Aliexpress unit would be the option of choice.
Cheapest option (and safest) is to just use the std. HS and fit a $1.29 30mm Pi Fan (with the four pads for spacing again) as discussed at length earlier ...

Update ...
I was going to try the Oven Bake repair method as a last resort to fix my Pro L, and gave it one last boot-up before consigning to the oven, well It booted fine !!
Must have been a couple of days rest that fixed it.
Could happen again no doubt but for now it is working again ...
(Note: I did try another damaged box with the Bake Repair method once before and it didn't worknever booted at all after being cooked for 10 minutes, so definitely a last resort)

Very quiet with a 100 Ohm, but I ended up using 117 Ohm for very near complete silence.
Using Sekisui Thermal Tape ?? seems OK.
Runs slightly warmer than the SZYTF Turbofan, but that fan is not fully silent, and may not start if more resistance is used to make it quieter !!
Temps (Ambient 20C)
Idle : 50C
YouTube HD : 57C
Kodi 4K60Hz : 64C
Stress CPU : 78C
Overall - a really nice bit of kit that will run very quietly.
Will do the job in all but the highest ambient temp environments with no noise at normal distances.
Thanks again to dbramm22 for sending this HS+Fan - very nice.

Cut one set of wires ...
Cut in Half (Cooler + Spare)
Two back legs are taller so swap the taller leg from the spare fan piece.
Gives a slight slope to the front ...
Tape up the wires to give bit of strain relief...
Do a better job than this !
Et voila, a little cooling plinth.
