I suspect a lot of these cheap Nextbook tablets have faulty NAND memory blocks, which don't always flash correctly. Unfortunately, it is unlikely that we will ever be able to do anything about this, because the only way to prevent this type of error would be to perform a low level reformat of the NAND to retest and mark the bad blocks so that they are skipped and never used again, and we don't have the tools to do this.
Also, people need to understand that "factory reset" doesn't work they way they expect it to. A factory reset only wipes data and cache. It doesn't do anything to the system partition because the system partition is assumed to be read only, which means it should never ever change, but of course many of us are rooting and then making the system partition read / write so we can add or remove system apps and edit the build.prop. If we make a change to the system partition that causes crashes or bricks the tablet, the only way to restore it is to flash a ROM.
